DOF clinches three-year AHTS deal with Total Austral

Norway’s DOF has been a awarded a three-year firm contract by Total Austral in Argentina for anchor handling tug supply vessel Skandi Pacific. The contract commences in the second quarter of 2017, and includes an additional two-year option. Investigatio into deadly accident found safety faults

A probe into the death of a man aboard the "Skandi Pacific" in the Pilbara found safety protocols aboard the ship were insufficient. The incident occurred in July 2015 as it was loading cargo containers from the Atwood Osprey oil rig in poor weather conditions. Due to the rough seas, the cargo transfer was stopped but two crew members, including 39-year-old seafarer Andrew Kelly, began to secure the cargo on the vessel in rough seas. While securing the cargo, the crew members slackened the securing chain they had used to secure the containers on the starboard side to better secure the entire stow. Early that morning, two large waves came over the vessel’s open stern, shifting the unsecured containers forward, crushing Kelly to death between the moving containers. The Australian Transport Safety Bureau investigation found risks associated with securing the cargo in the prevailing weather conditions had not been adequately assessed. The investigation identified that the "Skandi Pacific"’s safety management system procedures for securing cargo on deck in poor weather were inadequate, with no clearly defined weather limits. It also found there were no clearly defined limits for excessive water on deck, leaving individuals to make subjective decisions about whether or not to stop work and that "Skandi Pacific"’s managers had not adequately assessed the risks associated with waves coming over the vessel’s open stern when work was being undertaken on its aft deck.

Investigation into worker's death to wrap by June 2016

Australian authorities are investigating zjr fatal accident on the "Skandi Pacific" in heavy weather while working off Western Australia. The vessel was supporting semi-submersible drilling rig "Atwood Osprey" about 166 kilometres north-west of Karartha early on July 14 when the weather hit. The vessel stopped working cargo and attempted to take shelter. While the 39-year-old Andrew Kelly of Newcastle was trying to secure cargo, a wave came over the back deck of the vessel, crushing the man between a moving mini-container and a cargo skip. He was removed to the "Atwood Osprey" to receive medical assistance but died of his injuries. The offshore ship entered Dampier the same day and sailed again on July 16. The ATSB said the investigation, which will collect evidence from the ship and onshore, is expected to wrap by June 2016.
