Passenger fell four decks deep and died
The death of a passenger on board the 'Carnival Horizon' was investigated as it returned to Miami in the night of Nov 15, 2019. The passenger died soon after falling from a balcony on the ninth deck to the fifth, just as the ship was returning to Miami. There was panic and commotion when the man fell, as other passengers who converged on the scene blocked the view of others in higher decks. There was anxiety to know what was happening on the fifth deck. The crew tried to call a trauma helicopter to airlift the victim but he died before the medevac could take place. Carnival was working with the passenger’s family and cooperating with the Miami Police Department as they investigated. The vessel was on the last leg of a week-long cruise to Jamaica, Mexico, and the Caymans Islands.
Report: ineffective interaction and communication and ineffective oversight caused allision
A National Transport Safety Board report, dated Oct 22, 2019, stated that the probable cause of the 'Carnival Horizon' alliding with Pier 90, at the Manhattan Cruise Terminal, New York on Aug 28, 2018, was the ineffective interaction and communication between the master and the docking pilot who were manoeuvring the vessel, and the bridge team's ineffective oversight of the docking manoeuvre. Contributing to it was the placement of the third officer in a location without a view of the bow to monitor the close approach to Pier 90.
Tropical storm caused changes of itineraries
